
On Tuesday 26 January 2021, a fined of $340,000 was placed on an industrial gas supply firm and the managing director then was fined $45,000 for breach of workplace safety leading to an explosion sometime in 2015 taking the life of an employee and several injured.
An employee which just came back from her maternity leave a week before the incident was killed during the blast.
Last year Dec 1, both company and the managing director were charged with the offense under Workplace Safety and Health Act.
Apparently, they have failed to pass the check on their regulatory valve assemblies (RVA) which were modified, and also there is no tracking system for the accurate data of gas cylinder tracking leading to the explosion
