
A kidney patient who was receiving dialysis at Tan Tock Seng hospital died in 2016 from acute haemorrhage after an artery and a vein were accidentally punctured during the catheter insertion procedure.

It was believed that such a case was the first of its kind in Singapore. The patient’s name was Mr Lee Kuen Ngian and his death was ruled to be an unfortunate medical misadventure.
The doctor who reviewed Mr Lee was Dr See Yong Pey, who was a consultant at TTSH’s renal medicine department. Dr See most likely puncture the vessels unknowingly with a faulty technique.
The coroner said that there is no basis to suspect foul play in this case.
