
Singhealth have said today (24 Oct) that 111 patients and 6 staffs who received their vaccine shots from Bukit Merah Polyclinic between Oct 20 to Oct 22 were given lower dosages due to an error.
The dosages that they received was at 10 percent of what was the recommended dosage, and it happened because the markings that were on a new syringe which the clinic has just introduced was not identified correctly.
After the error was discovered, Singhealth immediately reached out to patients who are affected to determine the error’s extent.
All of them has been contacted and will be arranged to take a full dosage replacement vaccine, after being assessed by a doctor.
Singhealth also mentioned that based on the current vaccination guidelines by the Ministry of Health (MOH), it is unlikely that the reduced dosage that they receive will cause any negative reactions, and it is safe for them to get the replacement dosage offered.
Investigations by Singhealth also showed that this incident is a standalone, and the services at other polyclinics are not affected.
Actions have also been taken to ensure that this incident will not be repeated.
CEO of Singhealth polyclinic, Adrian Ee, also apologised for the inconvenience that this incident caused to the affected patients and their family members.
He added that staff have been educated on the correct procedures in administering the vaccine using the new syringe.
