Hanif Miah Md Nurul Islam, 39-years-old, was sentenced to 6 weeks imprisonment yesterday (24 February) after pleading guilty to knowingly using a forged certificate to gain employment as a formwork supervisor, according to MOM.
Hanif had used forged a “Formwork Safety Course for Supervisors” certificate to get the job, which involves making sure that the temporary moulds that were used for casting concrete structures were constructed, maintained and dismantled properly.
He had gotten the forged certificate for a fee back in 2015, and he was appointed a formwork supervisor 4 times between September and December 2020, based on the “qualifications” of his forged certificate.
MOM said that “in doing so, he falsely represented that he had completed the course, when he did not undergo the necessary training to be a formwork supervisor.”
Investigations into the authenticity of Hanif’s qualifications started in February 2021 after a sub-contractor contacted MOM for verification.
MOM added:
“Investigations revealed that Hanif had not received any safety and health training that was required for him to be familiar with the hazards associated with the construction, erection, alteration or dismantling of formwork.
Formwork operations are complex and dangerous, and poor installation or dismantling of formwork may result in the collapse of the entire formwork structure.
Hanif’s actions thus placed himself, workers under his supervision, and other workers at the worksite, at risk of serious harm.”
MOM also added that Hanif had forged certificates for a metal scaffolding erection course and a supervision of metal scaffolding erection course.
MOM further added that they will not hesitate to prosecute persons who forge their qualifications and safety credentials that endangers others.
