A 55-year-old heart attack patient was being transferred from Khoo Teck Puat Hospital (KTPH) to the National Heart Centre Singapore (NHCS) when the stretcher he was on collapsed head first onto the ground, with the man still attached to the stretcher.
He later died from a heart attack and bleeding in his brain.
A senior neurosurgeon said that although his head did not hit the ground directly, it contributed to the bleeding in his head.
What happened
The victim, 55-year-old Mr Razib, visited KTPH on 17 June 2018, after experiencing chest pains and he was later diagnosed to have suffered a heart attack.
He then underwent procedures to improve his condition but to no avail.
A medical team from NHCS in SGH, was brought in to aid in his heart function using a life support machine.
Mr Razib was due to be transported to NHCS but during the transfer, the battery of the life support machine went flat just as the team arrived at SGH.
But his pulse was still able to be felt and his condition was still stable.
He was then unloaded from the vehicle by the ambulance drivers and other NCHS staff (the KTPH porter who was supposed to assist in unloading the stretcher, couldn’t come along in the ambulance), but the stretcher’s collapsible wheels didn’t extend and click into position.
The portion of the stretcher that was supporting Mr Razib’s head fell and struck the floor, as his body was still secured on the frame.
He was transferred to the critical care area where the life support machine was powered up again and subsequently transferred tot he surgical ICU 2 h ours later.
A CT scan showed that he had bleeding in his brain and evidence of significant swelling and damage.
His condition was expected to deteriorate and his family decided to cut off life support.
Mr Razib passed away within an hour.
Death ruled a medical misadventure
On 13 April 2021, state coroner Kamala Ponnampalam ruled Mr Razib’s death as a medical misadventure, and that the head injury he sustained actually caused his condition to worsen.

A 24-year-old man from South Korea, Kim Tae Hyun, allegedly murdered 2 sisters and their mother at their home located at the northeastern part of Seoul.
The murders occured on 23 March 2021.
Kim pretended to be a delivery man to gain access into their home and at the time, only the younger sister was home.
So he killed her and waited for her sister and mother to come home before he killed them both by stabbing them.
Kim claimed that he met the younger sister through an online game, and that she refused to meet him, so he killed her.
Kim was reportedly turned down repeatedly by her and he became angry.
The girl also unwittingly revealed her home address while she was playing the game.
Kim was believed to have stalked his victims for months before murdering them.

Kim was reported to have stayed with the dead bodies of his victims for three days, eating and drinking alcohol at the murder scene.

A CCTV footage of his whereabouts 20 minutes before the killings was released, where Kim was seen at an internet cafe near the victims’ house.
He purportedly searched for “the fastest way to kill people” on his phone.

A Singaporean YouTuber, Sneaky Sushii, took a leaf out of MrBeast’s playbook and tipped delivery riders generously for their service.
He tipped a total of 3 delivery riders $50, $200 and $1000 respectively.
FIRST TIP
He first ordered a bubble tea and when his drink arrived, he tipped the rider $50 after complimenting him for his speed, and his reaction was one of disbelief.
SECOND TIP
He then ordered McDonalds for his 2nd order and when his food came, the rider even apologised for the “long wait”.
Sushii korkor then tipped the rider $200.
Although the 2nd rider’s reaction was somewhat muted and more composed, but he was more than happy to smile for the camera before leaving.
THE $1,000 TIP
Sushii then ordered some food for his third order, but he “forgot” to order a drink, so he called up his rider and asked him if he could help him buy a drink and pay for it first.
The rider, who was delivering on foot, obliged and when the order arrived, he told Sushii that he went to the market to get a can of Coke as it was only $0.90 as compared to the coffeeshop which sold it for about $1.
Sushii then lied that he doesn’t have small change, and he gave the hardworking rider a whopping $1,000 tip.
The delivery guy mind was completely Hiroshima-ed, as he told Sushii that the drink only cost less than a dollar.
He was visibly touched as it was his first time receiving such a huge and generous tip.
Well done bro, this guy deserves more subscribers than all those “10 Ways To”, “15 Types Of”, “369 Kinds Of” YouTube channels.
